What are they all about

Our Purpose is to develop a collaborative population health approach within the primary care sector. The primary health support service sector is often small and fragmented. Care Programs for complex and chronic needs are difficult to coordinate and agencies in the past have had to rely on their own planning strategies. The CWGPCP offers a unified approach to health planning and promotion within the Latrobe City and Baw Baw Shire through the cooperation of numerous agency members.

Our Focus is to create partnerships between the consumers and the GP's within the community, provide service coordination to improve information management and access to services, and create service planning to identify health and well-being needs and strategies to address needs based on integrated service strategies.

Membership of the CWGPCP includes the local government, Mental Health Services, Division of General Practitioners, Primary Care Providers, Latrobe Community Health Services, Co-Care Gippsland, Aged Care Assessment Service, WGHG, GAARSS, hospitals, and other agencies within the region.
